THRIVE GENERAL TERMS AND AGREEMENT (Website Consent)
By enrolling in Thrive membership services or using this website, you acknowledge and agree to the following terms:
Thrive is a membership-based care model. Thrive is not an insurance company and membership is not a substitute for health insurance. You are encouraged to maintain separate health insurance coverage.
Thrive provides primary care (Thrive Unity), gender-affirming care (Thrive and Thrive+), and wellness services (Thrive Care Wellness Add-Ons) as described on this website and in separate signed agreements. Not all services are included in membership. Emergency and hospital services are not provided.
Thrive reserves the right to change membership fees and service availability with notice.
Your membership may be canceled for non-payment, inappropriate behavior, or misuse of services.
Communications via email, text, phone, or web may not be fully secure. You accept this risk.
Thrive providers are not responsible for emergency care. For emergencies, dial 911.
The physician and/or Thrive team retains the right to decline or delay gender-affirming hormone therapy (GAHT) or related services if it is deemed medically unsafe or inappropriate for any member, based on professional medical judgment.
Thrive, its affiliates, providers, and team members are not responsible for individual dissatisfaction with outcomes related to any service provided, including but not limited to primary care, GAHT, aesthetics, or wellness treatments.
Use of Thrive membership and services constitutes acceptance of these general terms. A more detailed Patient Services Agreement will be provided and must be signed prior to receiving services.
